NDIS & SBH Queensland
The National Disability Insurance Scheme (NDIS) is a new way of providing individualised support for people with a disability, their families and carers. The scheme focuses on providing people who are eligible with a flexible, whole-of-life approach to support their needs so they can follow their goals and aspirations and better participate in daily life.

- We will advocate for your specific needs.
- We will support you when you receive your plan.
- We will provide you with allied health and social work services to help you reach your goals.
How to apply to be part of the NDIS
To be part of the NDIS, you need to meet eligibility criteria explained on the NDIS website.
Contact the National Disability Insurance Agency (NDIA) on 1800 800 110 and ask for an Access Request Form. SBH Queensland are happy to help you complete this form.
